How to join the team:

• Look for a contact form mid-April.

Please turn in the following forms on the first day of practice:

• Physical. Please print all pages and have the doctor fill them all out. Physicals must be dated 4/15/2024 or after to be valid for the 25-26 school year.

• Concussion Form. Please print out and send to practice with your athlete.

• Medical Release Form. Does not need to be notarized. Please print and send to practice with your athlete.

• Sudden Cardiac Arrest Acknowledgment Form. Please print and send to practice with your athlete.

• Media Release Form. Please print and send to practice with your athlete.
