In order to participate in Farragut High School Cross Country, the following forms must be filled out and turned into Coach Osborne. You may print out the non-Google forms and send them to Coach Osborne as a packet. You need to fill out the Google forms online.

  1. Physical Form. Please print all pages and have the doctor fill them all out. Send all pages back to Coach with the rest of these forms.

  2. Concussion Form Please print and send to practice with your athlete.

  3. Medical Release Form. Does not need to be notarized. Please print and send to practice with your athlete.

  4. Insurance Fee Acknowledgement and Link to pay insurance fee. Please print acknowledgement form and send to practice with your athlete.

  5. Sudden Cardiac Arrest Acknowledgement Form Please print and send to practice with your athlete.

  6. Media Release Form. Please print and send to practice with your athlete.
